Then package the equipment securely with your RAN clearly marked on the outside of the package, include this completed form inside your package, and return it to the address in the upper right corner of this form.

Cleaning of Material to be Returned

Thoroughly clean all material to be returned to Delta M. To minimize exposure risks to our evaluation/service team, any returned equipment must be thoroughly cleaned prior to shipping. If the equipment is soiled or contaminated on arrival, the equipment will be returned to the customer.
